Medical Update

To ensure we have current medical and emergency contact information for participants under the age of 18, we require an annual review of the information on file for your child. This information is essential to ensure participants can safely participate in activities. Your child will not be permitted to participate if the account information is not correct two-weeks prior to the start of an activity.

Please click the following link to access your account update medical information: SBParksAndRec.org/Signup.

After logging into your account, hover over the “My Profile” menu and click “Amend Medical Information."

Click the "Participant Name" box to select the child to enter medical information for. Please note that you will not see previously-entered medical information; you'll need to fill out this area again even if you've done it before.
