Every child should have the opportunity to experience the fun and joy of recreation programs. Inclusion services support children of all abilities who want to participate by working with families and program staff to understand each child's needs and identify reasonable accommodations.

Need an Accommodation?

We work with participants to identify reasonable accommodations based on individual needs and available resources. 

Plan Ahead

To allow sufficient time for staff consultation, participant assessment, activity evaluation, and coordination of support, please enroll in your desired program or activity at least three weeks before it begins and request accommodations within the medical waiver section while registering for activities. 


Registration Process
  1. Create a PerfectMind Account
  2. Enroll in your desired Program or Activity 
  3. Complete the Medical Questionnaire 
    Request accommodations through the Medical Questionnaire section and complete any required Supplemental Medical Forms. 
  4. Schedule an Assessment 
    Reach out to the Inclusion Coordinator to schedule a participant assessment. 
  5. Attend a Team Meeting
    A team meeting will be scheduled to discuss proactive strategies, and coordinate supports to help ensure a successful program experience.
